Remote patient monitoring allows a patient to use a mobile medical device to perform a routine test such as blood pressure and blood sugar checks. The date is then sent to our agency for evaluation in coordination with your physician. The test date is current so that any abnormal reading can be addressed immediately to avoid premature hospitalizations. Remote patient technology includes daily monitoring devices such as glucose meters for patients with diabetes and blood pressure monitors for patients receiving cardiac care. Remote patient monitoring is available for patients who require medical care for diabetes or hypertension and must qualify through medical necessity.
