Home health care includes skilled nursing care, as well as other skilled care services, such as physical therapy, occupational therapy, speech therapy, and medical social services. These services are available through our variety of skilled health care professionals at home. Our home health staff provides and helps coordinate the care and/or therapy your doctor orders and keeps your doctor and family up-to-date on your progress. Our goal in providing short-term home health care is to provide treatment for an illness or injury and to promote better health and independence.

Physical Therapy:
- Pain Management
- Home exercise
- Walker and cane safety instructions
- Massage therapy
- Patient Care Education on Activity of Daily Living
- Walking and transferring in/out of bed
Occupational Therapy:
- Developing patient and caregiver independence in self-care
- Services are given to help the patient return to usual activities such as bathing, preparing meals and housekeeping
- Bathroom Safety
Speech Therapy:
- Assistance to regain or restore speech skills
- Treatment of language disorders
- Treatment of swallowing disorders
Medical Social Worker:
- Social and emotional support related to your illness
- Help to access available community resources
- Home assessments to identify financial concerns of family and patient
Home Health Aide:
- Provides additional support to a licensed nurse
- Bathing
- Grooming
- Personal hygiene
- Toileting
All services are provided in accordance with the Medicare Certification standards.
