Long Term Care Division
Long-term care participants may include individuals with a disability, memory or thought processing problems and/or chronic physical illness. Long-term care is composed of many different types of services which may include assistance with activities of daily living. Through a safe-living environment and coordinated continuum of care, New Life Home Health is able to provide home health care based on individual needs, resulting in an increased quality of life and independence for our patients. Our goal is to enhance our patient’s quality of life and prevent unnecessary hospitalizations or nursing facility placements. Our attendants/providers have been trained to work diligently with our Nursing and Therapies staff for our more complex cases (post-hospitalizations, diabetes management and wound care patients.)
Comprehensive RN Assessment
Our team of registered nurses conduct comprehensive assessments to the patients, performing a head-to-toe evaluation which include physical or health examination. The information about the patients’ status on all aspects are then used to identify their current and future healthcare needs. This is to ensure that that they receive only the right type and level of care. During this process, our care team is able to draft a care plan tailor-fit to the clients’ health condition and preferences.
